Alpert et al. v. Optavia, LLC

You are part of the settlement if:

• - Subsequently canceled the program

What does the settlement provide?

Settlement Class Members can choose between:

• A $75 voucher for Optavia products (automatic if no action taken)
• A $70 cash payment (must submit claim form)

How do you get your payment?

For the voucher: You don't need to do anything. It will be automatically emailed to you once the settlement is final.

For cash payment: Submit a Cash Payment Election Form on this website by the deadline. You can choose to receive payment electronically or by check.

Important Dates:
